Information Release Form
You can find an Information Release Form on MyPage. In order to download it you need to:
1. Login to MyPage
2. Select Student Tab
3. Click on Academic Advising in the Student Portal
4. Click on Information Release form
5. Fill out the form and email to your program advisor. Go here for the list of program advisors' email addresses.
6. If you are a General Studies major or undecided, fill out the form and email to OnlineAdvisor@slcc.edu.
Students who want to allow information released to themselves or to an individual through email, must complete a declaration form providing a standing release to the College to disclose educational information. The information that may be released includes: progress toward completion of degree requirements, class scheduling, academic standing, grades, repeated courses and other similar information.
If you are a returning student, you may reactivate your MyPage account by creating a new password through SLCC MyPage. For additional information or if you are not a current student, please contact us at 801-957-4978.
Please note that our advisors are not able to email you the Information Release Form. Students help verify their identity by logging in and accessing the form via MyPage. When emailing an Online Advisor with your Information Release, please include your major and any specific questions you have in body of your email.
Please attach an Information Release to any email to any advisor when requesting personal academic information. Information Release are not stored so a release is required with each email communication.
The family Educational Rights and Privacy Act of 1974 (FERPA), initially called the Buckley Amendment, governs the release of records maintained by an institution of higher education and access to student records, including requests for information from parents, guardians, spouses, or others as designated by the student. Generally, without a student’s written consent, Salt Lake Community College may not disclose information from a student’s educational records.
