Forms and Documentation

You will need to fill out the DRC Application form, and contact the DRC at 801-957-4659 for an initial appointment to receive accommodations.

You can sign a Release of Information form to allow the DRC to share limited information on a need-to-know basis with faculty and others at SLCC.

Please submit the forms early to let us review your request and implement reasonable accommodations. You may be asked to provide additional documentation including diagnosis and functional limitations related to the diagnosis.

Students who do not have documentation are encouraged to meet with an Accessibility Advisor to explore accommodation options.

Documentation may include, but is not limited to:

  • Educational, psychological, or medical records.
  • Reports and assessments created by healthcare providers, psychologists, or educational professionals, such as a psycho-educational evaluation. (Neuro-Psych Evaluation or Psychological/Educational Evaluations)
  • Documents that reflect education and accommodation history, such as an Individual Education Program (IEP), Summary of Performance (SOP), and teacher observations.
  • Documents that includes a diagnosis and limitations associated with the diagnosis
