Disclosures of Consumer Information

Each year Salt Lake Community College publishes several disclosures of consumer information to the campus community. The purpose of this notice is to provide a listing of the available disclosures, their release date and where the campus community may obtain the information (668.41 (c)). This notice is published yearly to the campus community prior to July 1. Each of the disclosures can be found on the Salt Lake Community College web site at www.slcc.edu/disclosures.

1) Tuition and Other Institutional Charges and Financial Assistance Available to Enrolled Students

Interested individuals will find information concerning tuition and fee charges, estimates of books, supplies and transportation costs; refund policies, return of Title IV repayment policies; sample loan repayment schedules, general terms and conditions of work study employment, student loan exit counseling and deferment information procedures for officially withdrawing from the institution.  Paper copies of this information are also available at the Office of Financial Aid.

2) Institution Information that Pertains to Academic Programs and Accreditation

This information is published yearly in the Official College Catalog.  The catalog provides information on current programs, physical facilities, faculty, accreditation, study abroad programs and special facilities and services available to disabled students.  Catalogs are available through the Office of Academic Advising, Enrollment Services and the Campus Bookstore.

3) Institution’s Completion or Graduation Rates

Salt Lake Community College presents its student completion, graduation and transfer rates of a yearly cohort group of students. This information is published yearly by July 1.  Paper copies are available upon request from the Office of Enrollment Services.

4) Institutional Security Policies and Crime Statistics

In this annual report, Salt Lake Community College provides information regarding the frequency of crimes committed on its campuses and policies and procedures designed to combat crime and strengthen security for the three most recent calendar years. The report includes, current policies and procedures for students and employees to report a crime, a list of the titles of those to whom a crime is reported, the College’s relationship with local and state police agencies, a statement of the drug and alcohol policies, the type and frequency of programs designed to provide information on campus security procedures and policies, information on crime prevention and on sexual assault programs, support and procedures regarding sexual assaults.

This report complies with the requirements of the “Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act.”  This information is published yearly by October 15.  Paper reports are available upon request from the Salt Lake Community College offices of Security, Human Resources and Enrollment Services.

5) Information on Athletic Program Participation rates and Financial Data

SLCC's Athletic Program Participation Rates and Financial Data is now maintained at http://ope.ed.gov/athletics.  To view the data, please click the link and select, "Get data from one institution" on the right side, and then search for SLCC.

This disclosure charts the expenses, revenues and support provided to intercollegiate men and women’s athletic programs.  This information is published yearly by October 15.  This information is available in print from the SLCC Athletic Department upon request at 4600 South Redwood Road, Salt Lake City,  UT 84130-0808.

6) Rights Under the Family Education Rights and Privacy Act (FERPA)

This notice is published yearly in the Official SLCC College Catalog.  It presents a student’s rights and responsibility with regard to their academic records. 

7) Alcohol and Drug Campus Policy

Salt Lake Community College publishes yearly its Alcohol and Drug Campus Policy in each official Class Schedule. This policy describes applicable alcohol and drug laws and their penalties; alcohol health risks and locations students may go for help.
