TRANSCRIPTS

The permanent record (transcript) of your academic achievement is maintained by Enrollment Services.  Transcript requests can be processed at Redwood Campus, South City Campus, Jordan Campus, and Sandy Center.  For information, please call (801) 957-4297. 

View Your Transcript Online -- No Charge

You may do the following to view an unofficial transcript online:

• Go to www.slcc.edu and login to the MyPage login box on the lower left
• Enter your username and password (PIN) (If you don't have your username/password, click on the "Need Your User/Pass" link in the MyPage login box)
• Once you are logged in, locate and click on the Student tab in the upper left
• In the Services for Students channel, locate Student Records, and click on Academic Transcript
• Select the Transcript Level and Type and click Submit

Order Official Transcripts

All official transcripts are mailed and cost $4.00 per copy. To order an official transcript you may choose one of the following options:

Order Your Official Transcripts Online

1. Sign into MyPage* and click on the Student tab
   *We can not give out the student number or login information via telephone.
2. In the Services for Students channel, locate Student Records, and click on Order Official Transcripts
3. Have your Credit/Debit card ready. You will be asked to make your transcript fee payment during the ordering process.
4. Follow the prompts until the request is complete.

Transcripts ordered online are usually processed within 24 business hours, excluding holidays and school breaks.

In Person

1. Complete a Transcript Request Form at Enrollment Services
2. Pay $4.00 per copy at Student Center Courtesy Desk or Cashiers Office
3. Bring completed request form to Enrollment Services with picture ID

By FAX and Credit Card

1. Call cashier at (801) 957-4868 to pay fee by credit card.
2. FAX request to (801) 957-4961 with the following information:

• Name
• Student ID or Social Security Number
• Date of Birth
• Last Date attended
• Previous Names Used
• Name and address of person or organization transcript is to be sent to
• Student Signature
• Current Contact Information

By Mail

Mail a completed Transcript Request with a $4.00 check to:

Faxed and mailed transcripts are usually processed within 3-5 of receiving your request.
Please do not submit transcript requests on scroll paper.

VERIFICATION OF ENROLLMENT

Salt Lake Community College has authorized the National Student Clearinghouse to act as it’s agent for verifications of student enrollment.  Please contact the Clearinghouse via e-mail at enrollmentverify@studentclearinghouse.org, by telephone at (703) 742-4200, or by FAX at (703) 742-4239.  Verifications requests are available through Student Self Service.

Please contact Enrollment Services for health, automobile, good-driver discount, or other insurance verifications.  Picture ID is required.
