With the exception of those forms which require an appointment, all forms and their supporting documents may be submitted in person at the Financial Aid Office, by Fax to (801) 957-4657, or by mail. Incomplete documents will not be processed. Please allow five working days for documents to be processed and then contact our office at (801) 957-4410.

Choosing the Lender for your Student Loan There have been important changes in student loans in the past year. Please read this document.

ACG Requirements Document Use this form if you have been told you may qualify for the ACG.

2009-2010 Stafford Loan Acceptance Form Use this form to accept an awarded Stafford Loan.

2009-2010 Stafford Loan Application Form Use this form only if you have not already been awarded a Stafford loan or you need to request a change to your Stafford loan.

2009-2010 Update Form Use this from to change your enrollment status or make us aware of anything else you need to tell us.

2009-2010 Additional Unsubsidized Stafford Loan Application Students may use this form to request additional Stafford Loan funding. You must make an appointment with a Financial Aid Advisor to submit this form.

2009-2010 PLUS Application This form should be used by dependent students and their parents to request a PLUS loan.

Appeals Forms

These forms are used to request exceptions to financial aid policies.


2009-2010 Appeal for Independent Status Students who do not qualify for independent status on the federal financial aid application may request a review of their status. Students wishing to be considered independent must demonstrate that they have unusual circumstances.

2009-2010 Special Consideration Request Students who have experienced a significant change in their financial status may request special consideration. Students must meet with a Financial Aid Advisor to submit this form.

Appeal for Continuation of Funding, Part Two Students who have been informed that they are at or near the maximum number of allowed credit hours must complete and submit the above form.  You must see an Academic Advisor to complete Part 1 of the Appeal for Continuation of Funding.  Parts one and two must be complete before an appeal can be considered.

Satisfactory Academic Progress Appeal Form This form is used to request exceptions to various policies.

Selective Service Appeal Form Students who have been instructed that they need to do an appeal regarding Selective Service registration are to use this form and follow the instructions on the form.
