Apply for admission at SLCC (if you have not attended SLCC within the last three years). Declare your major as Pre-Medical & Health Sciences. Schedule an appointment to meet early with an Academic Advisor to make sure you’re on track to complete all program prerequisites.

Prerequisite Courses

All prerequisite courses (or equivalent transfer credit, AP or CLEP) must be completed by the Advanced Placement Nursing Program application deadline.

• Students taking courses at SLCC, must complete all courses that will be considered on your application by August 31 if you plan to apply for spring, December 31 if you plan to apply for summer, and May 31 if you plan to apply for fall.  Transfer students must be complete by May 31 and have transcripts sent to the Transfer Evaluation Office by August 1 if you plan to apply for spring. Transfer students must be complete by August 31 and have transccripts sent to the Transfer Evaluation Office by October 30 if you plan to apply for summer. Transfer students must be complete by December 31 and have transcripts sent to the Transfer Evaluation Office by March 15 if you plan to apply for fall.

Each course and lab must be completed, with a C grade or better, except the Pathophysiology course which must be completed with a C+ or better. Courses that have not been completed with a C grade or better (or a C+ for Patho) will not be considered.

IMPORTANT NOTES ABOUT TRANSFER CREDIT

• It is the applicant's responsibility to make sure their transfer credit has been fully transferred to SLCC prior to the  deadlines.
• Transcripts must be sent to the Transfer Evaluation Office by March 15 for fall sessions, August 1 for spring sessions and October 30 for summer sessions. Grades must be posted to the transcripts at the time they are sent to the TEO.
• Students should declare their major as Pre-Medical & Health Sciences and specify which Health Sciences program they will be applying to when they submit their transfer evaluation request. If the correct information is not submitted within the transfer evaluation request, courses applicable to the program may not be transferred to SLCC.

VERIFICATION OF HIGH SCHOOL GRADUATION OR EQUIVALENT (GED)

Verification of high school graduation or equivalent (GED) will be required before the (program) admissions deadline. Send official high school transcripts to HighSchoolTranscripts@slcc.edu.

Unencumbered Utah LPN License

Your LPN license must be current and in good standing in the State of Utah. You must submit a copy of your LPN license (that clearly shows your LPN license number) to HealthSciencesAdmissions@slcc.edu by the program's application deadline.

Apply to the Nursing, Advanced Placement program through your MySLCC account:

• MySLCC
• Click on "Discover" (Found on the top left under the 3 horizontal lines next to the SLCC logo)
• Type in "Selective Admissions Forms
• Click on "Health Science Program Admissions"
• Nursing

(Nursing, Advanced Placement will only appear in menu during open application periods.)

Acceptances into the Nursing program will be given on a "conditional" basis.  Students must complete the following within the allotted time in their acceptance letter to receive a full acceptance:

1. Order Complio compliance package.
2. Order background check.
3. Upload and enter documentation showing compliance for immunizations.
4. Complete drug screening (directions will be provided in the conditional acceptance letter)
5. Complete American Heart Association or American Red Cross BLS Healthcare Provider CPR certification.

Once all of the above items have been completed, the student will need to contact the Admissions Office for verification of compliance.  If compliant, a new acceptance letter will be sent with additional instructions for course registration, orientation information, books to order, etc.  Clinical placements for third semester will be released to students no later than the first week of the semester.

Please do not initiate any of the compliance tasks unless you have received a conditional acceptance letter.

Background Check and Compliance Package

Students who do not receive favorable background checks, may not be able to sit for national certification exams or meet the requirements for clinical placement.  Students who do not receive favorable background checks should contact admissions immediately.  An unfavorable background check may prevent acceptance into the program. The School of Health Sciences strongly recommends that prospective students take measures to have criminal offenses (felonies and or misdemeanors) expunged if possible.  Information regarding how to expunge criminal records may be found here:  https://www.utcourts.gov/howto/expunge/

To order Criminal Background Check and Compliance Packages:

1. Go to Complio
2. Follow the instructions to set up an account.
3. Order the Criminal Background Check Package ($29.00) and the Compliance (Nursing package) Package ($15.00). The compliance package is a 12 month subscription that needs to be renewed every 12 months while you are in the program at a cost of $15.00.

Immunizations

Proof of immunization is required by clinical affiliates and must be completed upon acceptance into the program. Clinical affiliates mandate all students comply with program immunization requirements, as failure to immunize is not only a risk to patients but also students. Failure to immunize may result in the inability to place students at clinical sites, which would also preclude students from completing required clinical coursework.  Students that cannot comply may forfeit their placement in the program. Students claiming a religious or personal exemption for an immunization should fill out this form (link) and provide to the Associate Dean of the specific program.  Students claiming an allergy to an immunization should complete this form (link) and provide to the Admissions Coordinator in the Dean’s Office.  The School of Health Sciences does not guarantee that clinical affiliates will accept exemption requests.

 In your Compliance Package, you must provide proof of the following immunizations:

1. Annual Influenza Vaccination:
   Generally given September-February. Students must provide proof of the Influenza vaccination or submit a signed declination.
2. Turberculosis Blood Test: Required Annually
   Proof of a negative 2-step PPD or QuantiFERON TB Gold test. If positive or if a past test was positive, student must submit a negative chest x-ray. This requirement also includes international students who have had the BCG immunization. X-ray results are good for one year.
3. Tetanus, Diphtheria and Pertussis (Tdap) Booster:
   Tdap booster is good for 10 years. TD will be given subsequent to Tdap. If it has been more than 1 1/2 years since receiving TD, the student must have their tetanus updated with a Tdap.
4. Measles, Mumps, Rubella (MMR):
   Two doses are needed with booster as an adult or positive titer on all three. Equivocal results are not acceptable; students must test in the positive range. NOTE: If given to adult females, since this is a live virus, you cannot be pregnant at the time you receive the immunization and you must avoid pregnancy for 3 months after administration
5. Varicella (Chicken Pox):
   Two immunizations or proof of positive titer. Stating you had the disease is not acceptable. Varicella immunizations are expensive, if a student had the disease it is recommended to start with a less expensive titer first for proof of immunity before starting the immunizations.
6. Hepatitis B vaccination series:
   Must provide proof of a HEP B Titer (blood test) that proves immunity.
7. Hepatitis A vaccination series: Highly recommended but not required
   Two dose series or a positive titer. If a student starts the series, they must complete it.

The SLCC Center for Health and Counseling offers immunizations at a discounted rate.
Some clinical facilities may require additional immunizations.

CPR Certification

American Heart Association Two-Year Health Care Providers or American Red Cross BLS Healthcare Provider will be accepted. Must be kept current throughout the duration of the program.
