If your card is lost or stolen you should report it immediately to one of the ID centers located at Redwood Campus (957-4022), South City Campus (957-3407), or Jordan Campus (957-2600) and Sandy Campus (957-5890). Cards reported lost or stolen will be deactivated, and it only takes seconds to deactivate your account. All purchases debited to the account prior to notification are the responsibility of the cardholder. Upon notification a replacement card can then be issued ($15).

Replacement Cards are issues for damaged, lost or stolen cards at any ID center. The option of taking a new picture will be offered at that time. Replacement fee for lost, stolen or mutilated cards is $15. When you initially come into either ID center ask to have the lost & found file checked, your card may be there waiting for you. The fee will be waived if the card was damaged, but not intentionally mutilated, you must bring your card in with you. You may also receive a waiver of the fee if your card was stolen and you bring in a copy of a filed police report. Once replaced, the old OneCard cannot be reactivated or reused. If you find the old card please destroy it so that you don't confuse the two cards.
