ENGL 1010 Intro to Writing


ACT/SAT/Accuplacer exam placement into ENGL 1015 or above within the past year


MATH 1010 Interm Algebra, or higher within 5 years of application

PSY 1100 Human Growth & Dev 3
BIOL 2320 Human Anatomy 4
BIOL 2325 Human Anatomy Lab 0
COMM 1010 Elem Effective Comm 3
OTA 1020 Intro to Occupational Therapy 2

- Note: All prerequisites must be completed BEFORE applying to the Occupational Therapy Assistant program.The following prerequisites MUST be completed with a B grade or better within the past 5 years: BIOL 2320, BIOL 2325, and OTA 1020 (OTA 1020 can only be taken twice). MATH 1010 or higher must also be taken within 5 years of the application. The remaining prerequisites must be completed with a C grade or better. See application deadlines. It is the student's responsibility to examine each course description for details of other prerequisite classes

Health Professions Admission Assessment
- Contact the Health Sciences Assessment Center to schedule an exam at least 1 week BEFORE you plan to take the exam
- A minimum overall score of 80% or better is required. The exam score is valid for 2 years. If the minimum score is not achieved on the 1st attempt, a 2nd attempt is allowed. Applicants are required to remediate for a minimum of 2 weeks before the 2nd attempt. The highest score of the two attempts will be used; there is no averaging of the two scores. The areas tested include: Anatomy & Physiology, Biology, Math, Grammar, Reading Comprehension, and Vocabulary & Gen. Knowledge. Learning Style and a Personality Profile are also included (for information only). Exam must be scheduled at least one week in advance

Immunization Form and CPR/First Aid Certification
- Have a health care provider fill out and sign the Immunization Form
- Bring a current CPR and First Aid Certification to the Admissions Office. CPR and First Aid Certification must be one of the following: American Heart Association two-year Health Care Provider, or American Red Cross two-year for Professional Rescuers and Health Care Providers (Red Cross certificate ID will need to be submitted for verification).
SLCC Health & Wellness Services offers immunizations at a discounted rate
*Some clinical facilities may require additional immunizations
**Student assumes the risk if he/she does not have the required immunizations and the clinical site(s) do not accept him/her, he/she may not be able to complete the course and the program. This may also result in possible withdrawal from the program after the tuition refund deadline is past

Background Check
- Order a background check by going to www.certifiedbackground.com and click on "students" (package code: SD22)
- Pay background check fee: $45

*Student assumes the risk if their criminal background is not clear, he/she may not be able to complete the course and the program. This may also result in possible withdrawal from the program after the tuition refund deadline is past. Only one placement attempt will be made per clinical course (this may depend upon the program). Students are advised to contact Deb Morrill, the Program Specialist at deb.morrill@slcc.edu or 801-957-6203 for any questions or concerns prior to applying to a program.

Background information:  Students who have committed a misdemeanor or felony involving moral turpitude may not be eligible to participate in service learning, fieldwork, state licensure, or national certification.

**Background check is good for one year from order date.

Community Service
- Completed 25 hours of community service
- Community service should NOT be occupational therapy related
- Fill out the Community Service Verification Form and submit one letter of recommendation

The following will instruct you on how to access the Occupational Therapy Assistant Application:
- Log into MyPage
- Click on the Student Tab
- In the Health Sciences Channel box, click on Program Admissions to access the application
- Application fee: $25

Proctored Essay
- After submission of the Occupational Therapy Assistant Application, you may be contacted to complete a proctored essay
- Students will be notified of a specific date for the proctored essay.

Notification of Acceptance or Non-Acceptance
- Notification will be sent to student's Bruinmail
- If accepted, further instructions regarding the drug screening and orientation will be in the notification of acceptance
- Drug screen fee: $35
- Orientation fee: $5
