Complete COVID Vaccination Form

1. Log into MySLCC and select Registration tab and select URL for COVID vaccination form.

2. Complete and submit form.

How to attach forms
  1. Select attachment icon.
  2. Upload file.
  3. Click on submit.

Note: Refresh browser after submitting form. If you are using College Scheduler you will need to logout completely, and then log back in.

3. Return to Registration tab. When vaccination status is green, you may select registration tool and register for classes.

How to View Holds

1. Review hold on Academics & Records tab. Click on view holds link on Academic Records channel.
