Dental Benefits
SLCC is making available a unique dental benefit that can cover you, your dependents and family. There is no premium to participate in the Dental Benefit Program and participating dentists have agreed to an average 20 percent discount on services. To participate, you'll need an activated Dental Benefit card, which you must present at the time of service.To enroll and receive your activated card:
- Visit Dental Benefit Program
- Select Get Started No Cost
- Select I have received a Group ID # only . . .
- Using Group ID # SLCC100, enter your information in the fields
- Once activated, you'll be assigned a Patient ID # that you should write down and remember
- Your permanent dental benefits card will be mailed in 3-5 business days
You can view participating dentists and the discounted fee schedule by logging in with your Group ID # and Patient ID # at Dental Benefit Program.
If you need to see a dentist before your Dental Benefit card arrives, please give the participating dentist your Group ID # and Patient ID #.
Your dental benefit card can only be used at participating Dental Benefit Program dental practices. Payment arrangements must be made with the dentist at the time of service. Participating dentists offer full dental services, including cosmetics, implants, bridges, crowns, restorations, dentures, root canals, and teeth whitening. If you have any questions regarding costs, please contact the participating dentist. The program includes dental specialists.
