DRC Documentation Guide

Students should contact Accessibility and Disability Services for documentation guidance and provide the most current documentation they have. Depending on the disability more documentation may be needed. The Accessibility and Disability Services will use any relevant source of information when making determinations about eligibility and accommodations. The use of previous services and related documentation such as IEP's and 504's will be taken into consideration when determining eligibility.

Overview

Documentation should include a description of the disability and its current impact in an educational setting and include the following:

  • Be from a licensed professional such as a medical doctor, audiologist, psychologist, or therapist/counselor who is familiar with the student’s disability and its impact in a university setting.
  • Writing, on letterhead, with the name, and provider credentials is preferred.
  • The medical care professional providing documentation cannot be related to the student.
  • Clearly state the nature of the disability.
  • Provide clear details on how the disability substantially limits major life activities, focusing on activities in an educational environment. Major life activities include, but are not limited to, caring for oneself, performing manual tasks, seeing, hearing eating, sleeping, walking, standing, lifting, bending, speaking, breathing, learning, reading, concentrating, thinking, communicating, and working.

Please Note

  • Accessibility and Disability Services may ask the student to provide additional information if the submitted documentation is incomplete or does not support the student's request for accommodations. 
  • Students who do not have documentation are encouraged to meet with Accessibility and Disability Services .
  • Students should keep the original copy of the documentation for their personal records. Documentation submitted to the Disability Resource Center will not be returned to the student.
  • Documentation accepted by Salt Lake Community College might not be accepted by other institutions, agencies, and/or programs (e.g., testing agencies, licensure exams, and other colleges or universities). Please check with the specific organizations and/or programs to determine their documentation requirements.
  • Accessibility and Disability Services will determine reasonable accommodations at SLCC.
