Send Your SLCC Accuplacer scores to another school

You may access your Accuplacer scores through Accuplacer’s student portal.

Complete the Accuplacer Test Scores Release Form (PDF) to have your scores sent to another institution.

Please print and sign the form. We must have your handwritten signature.

Email it to testingservices@slcc.edu or fax it to 801-957-4750.

Allow 2 business days for processing.
