REQUEST A TUTOR - please read all the instructions
NOTE:
We will begin accepting Tutor Request forms for Spring 2009 on
January 5th,
one week before school starts.
Tutor Requests turned in before that time will be returned or discarded.
Tutor Requests will generally be accepted until one month before the last day of class; however, matches can only be made if there are still available openings.
STUDENTS NEW TO FOCUSED TUTORING
- Fill out the New Student Tutor Request Form.
- See an advisor and complete the attached Course Plan listing the courses you need to complete your degree.
- Turn in the Tutor Request Form at TB 213, mailbox for Jennifer Fasy.
- Sign up for an Orientation outside TB 506A.
- Attend the Orientation and sign your tutoring agreement. Request forms will NOT be processed without:
a. Course Subject and Number
b. Availability Schedule
c. Course Plan
Returning students do not need to fill out another Course Plan and they do not need to attend another Orientation. However, returning students are still committed to abide by the agreements they made during their Orientation.
Returning Students Tutor Request Form (This form is almost exactly like the New Student form except it does not have the Course Plan attached so you can print less paper.)
