Information Sheet Form with VA contact information, monthly certification procedures and list of documents needed to successfully apply for benefits.

Advanced Pay Request Form Complete if you are requesting advance pay (First 2 months of benefit up front).  Complete and submit no later than 30 days prior to the beginning of the semester in question.  You must have had at least a 30-day break between periods that you were paid benefits to use Advance Pay.

SLCC Request for Benefits  Notifies the SLCC Veterans Office of your intent to use VA benefits, provide updates, contact information and release of information authorization concerning your benefits.  *Under the Family Educational Rights and Privacy Act (FERPA), the Veterans Office is not authorized to release any students' private information without express written consent by the student or legal guardian.*
