Applications for admission to the Physical Therapist Assistant program are only accepted during open submission periods. Please see the School of Health Sciences Application Submission Dates for current submission dates.

The Physical Therapist Assistant program uses a merit-based, competitive admission process. We accept one cohort of 24 students every Fall semester.

If you are accepted to a program a confirmation email will be required, please read all emails carefully and thoroughly to make sure you don't miss any information.

Proof of immunization is required by clinical affiliates and must be completed upon acceptance into the program. Clinical affiliates mandate all students comply with program immunization requirements, as failure to immunize is not only a risk to patients but also students. Failure to immunize may result in the inability to place students at clinical sites, which would also preclude students from completing required clinical coursework.  Students that cannot comply may forfeit their placement in the program. Students claiming a religious exemption for an immunization should fill out this form (link) and provide to the Associate Dean of the specific program. Students claiming an allergy to an immunization should complete this form (link) and provide to the Associate Dean of the specific program.  The School of Health Sciences does not guarantee that clinical affiliates will accept exemption requests.

Students who do not receive favorable background checks, may not be able to sit for national certification exams or meet the requirements for clinical placement.  Students who do not receive favorable background checks should contact the program immediately.  An unfavorable background check may prevent acceptance into the program. The School of Health Sciences strongly recommends that prospective students take measures to have criminal offenses (felonies and or misdemeanors) expunged if possible.  Information regarding how to expunge criminal records may be found here:

Apply for admission at SLCC (if you have not attended SLCC within the last three years).

Declare your major as Pre-Medical & Health Sciences. Meet early with an Academic Advisor to ensure that all prerequisites and submission requirements are completed before applying to the Physical Therapist Assistant program.

Prerequisite Courses

Please Note: Students who plan to apply for admission to the Physical Therapist Assistant program, for entry beginning in Fall semester 2020, must take MA 1100 Medical Terminology in place of PTA 1500 Practice Issues for the PTA. PTA 1500 will no longer be offered.

All prerequisites must be completed before applying to the Physical Therapist Assistant program, or if currently enrolled in a prerequisite course, official grades must be posted to your SLCC Degree Works transcript before the application deadline. However, if you are waiting for transcript grades you can still submit your application materials and then add the grades as they are available prior to the deadline.

All prerequisite courses have a limit of two attempts every 5 years. BIOL 2320/2325 and PTA 1010 must be completed with a B grade or better, and must have been completed within five years of the start of the program. The remaining prerequisites must be completed with a C grade or better.

  • It is the applicant's responsibility to make sure their transfer credit has been fully transferred to SLCC prior to the program's application deadlines.
  • If a student has courses they are transferring from another school, their transcripts must be evaluated by the SLCC Transfer Evaluation Office and their grades must be posted to SLCC DegreeWorks (their MySLCC account), before the program’s application deadline (priority and final application deadlines apply). IT CAN TAKE SEVERAL WEEKS FOR TRANSCRIPTS TO BE EVALUATED.
  • Students should declare their major as Pre-Medical & Health Sciences and specify which Health Sciences program they will be applying to when they submit their transfer evaluation request. If the correct information is not submitted within the transfer evaluation request, courses applicable to the program may not be transferred to SLCC.

HESI Health Sciences Pre-Admission Exam

  • Tests must be scheduled at least one week in advance.
  • You must pass with a minimum overall score of 75%. 
  • To raise your score, you may retake the exam after a two week remediation period. You may take the exam a total of two times per two year period.
  • HESI scores will be valid for two years.

The proctored essay is required for admission purposes, but will not be offered until sometime after the program's application deadline. The proctored essay will be held in the Jordan Campus Health Sciences Assessment Center, JHS 054. Applicants who fulfill all of the program application requirements by the application deadline will receive an email (after all applications have been reviewed) that contains more details about the proctored essay

Applicants who submit documentation showing they have earned an Athletic Trainer degree may be given extra consideration for the program, if they have fulfilled all of the program application requirements.

Background Check and Compliance Package

Beginning Fall 2019, purchasing a Complio account/Background check and fulfilling the immunization/CPR requirements, will no longer be required for admission purposes.  If you are accepted into the program, you will need to purchase both the Compliance Package account and the Criminal Background Check AND begin working towards fulfilling all of the immunization/CPR requirements as soon as possible.  **The Hepatitis B immunization requirements can take 6 months or longer to complete.

To order Criminal Background Check and Compliance Packages:

  1. Go to Complio - background and compliance management system.
  2. Follow the instructions to set up an account.
  3. Order the Criminal Background Check Package ($29.00) and the Compliance Package ($15.00). The compliance package is a 12 months subscription and will need to be renewed every 12 months you are in the program at a cost of $15.00.

You assume the risk if your criminal background check is not clear. If you do not pass the background check, you may not be accepted into the program, which may result in withdrawal from the program after the tuition refund deadline has passed.

It is strongly recommended that prospective PTA students take measures to have criminal offenses (felonies and or misdemeanors) expunged or sealed from your record if possible.


Do not order the Drug Screening Package ($35.00) at this time. We will notify you at a later date to purchase a Drug Screening Package. If you go ahead and order it at this time, SLCC will not reimburse you and you will have to order and pay for it again at a later date. Once you have purchased the drug screening package and been notified you will only have 24 hours to complete the test.

The Background Check, Compliance Package and Drug Screening must be completed before you begin your internship.



In your Compliance Package, you must provide proof of the following immunizations:

  1. Annual Influenza Vaccination:
    Generally given September-February. You must provide proof of the influenza vaccination or submit a signed declination. This is not required for admission, but will be required once you are in the program. (You may be required by a clinical site to have the influenza vaccination)
  2. Tuberculosis Blood Test: Required Annually
    Proof of a negative QuantiFERON TB Gold test or T-SPOT.TB; if positive, you must submit a chest x-ray read by a radiologist with a normal result; if the chest x-ray is abnormal, you must be cleared by your physician or local health department. This requirement also includes international students who have had the BCG immunization. Once in the program an annual Quantiferon will be required. The need for a repeat chest x-ray will be done on a case-by-case basis.
  3. Tetanus, Diphtheria and Pertussis (Tdap) Booster:
    Tdap booster is good for 10 years. TD will be given subsequent to Tdap. 
  4. Measles, Mumps, Rubella (MMR):
    Proof of two doses or a positive titer (blood draw) on all three (measles, mumps and rubella). Equivocal results are not acceptable; you must test in the positive range if you choose to do a titer.  NOTE: If given to adult females, since this is a live virus, you cannot be pregnant at the time you receive the immunization and you must avoid pregnancy for 3 months after administration.
  5. Varicella (Chicken Pox):
    Proof of two doses or a positive titer (blood draw). Stating you had the disease is not acceptable. Varicella immunizations are expensive, if you had the disease it is recommended to start with a less expensive titer first for proof of immunity before starting the immunizations. Once in the program if titer was negative, student must submit 2 doses of the vaccine taken 30 days apart and 30 days after the negative titer. Documentation of the two doses is due in Complio by August 1st.
  6. Hepatitis B vaccination series:
    You must submit a positive Hepatitis B titer. Once in the program, if the titer was negative or equivocal – 1) Submit the dates of your 3 dose series of shots in Complio, 2) Reinitiate the immunization series immediately – repeat shot #1 due by July 1st, shot #2 due by August 1st, shot #3 due by March 1st, and repeat titer due by May 1st.
  7. Hepatitis A vaccination series: Highly recommended but not required
    Proof of a two dose series or a positive titer. If you start the series, you must complete it.

The SLCC Center for Health and Counseling offers immunizations at a discounted rate for students who are low income. For a discounted rate, take your previous income tax return or your last 30 days pay stubs.
Note that some clinical facilities may require additional immunizations.

Student Health Form: This is not considered in the selection criteria but must be submitted to the Health Sciences Admissions office in person or by email at Do not upload this form to your Complio account. You need to submit documents from your doctor/clinic that show the date of your immunizations and the results, they must include your name as well as the doctor/clinic name. CPR Certification

American Heart Association Healthcare Provider (BLS Provider) card valid 2 years through the month of December graduation are due in Complio by the application deadline.

BLS Provider

The new BLS Course replaces the BLS for Healthcare Providers (BLS HCP) and BLS for Prehospital Providers (BLS PHP) courses. The BLS Course teaches both single-rescuer and team basic life support skills for application in both in- and out-of-hospital settings. This course trains participants to promptly recognize several life-threatening emergencies, give high-quality chest compressions, deliver appropriate ventilations, and provide early use of an AED. It includes adult, child, and infant rescue techniques. Provider cards are available exclusively to authorized TCs for issuance in accordance with the AKA policy.

BLS Healthcare Provider

The BLS for Healthcare providers and BLS for Prehospital Providers Courses have been replaced by the 2016 BLS Course.

Admission Guide for Student Selection

Selection is based on points in the following areas:

  • Pre-requisite course grades above the minimum requirement.
  • HESI exam score above the minimum requirement.
  • Essay meeting the following expectations: introduction and conclusion relating to topics provided on essay prompt, content, organization of content, examples to support content, and language and mechanics.
  • Previous Athletic Training degree is helpful but not required.
