Consent for Use of OneCard/Mastercard

I authorize Salt Lake Community College to deliver any credit balance, including but not limited to Federal financial aid funds or payroll, electronically to the Money Network® enabled Mastercard account, which is part of my SLCC student ID card.

SLCC reserves the right to recall or adjust any deposits improperly created and deposited to this account. I authorize any such recall/adjustment requests made by SLCC.

By consenting to use the SLCC OneCard/Mastercard to receive your refund, you agree to:

• Allow SLCC to transmit required personal data to a 3rd party for the creation of your account and for distribution of funds to your OneCard Account.

• Fee Schedule
• Initial card is FREE; replacement cards cost $20.

The account will not incur any charges as long as you use In-Network ATMs, money network checks, or use the card anywhere that Mastercards are accepted and in accordance with the rules outlined in the Fee Schedule.

I confirm that I have read the Terms & Conditions of the program and I understand that I have the right to withdraw my participation in the program at any time by providing written notice to the SLCC OneCard Office Located at the Taylorsville Redwood Campus Student Center 021.
