Patient Portal

Welcome to the Patient Portal

Once logged in, you will have access to the following:

  • Complete health history forms
  • Lab results
  • Educational health information
  • Secure communication from your provider

Visit the Patient Portal

Are you an existing user? If so, log in with your username and password.

New to the Portal?

  1. Click Visit the Patient Portal above. You will then be transferred to a new webpage.
  2. Click Register at the top right corner of the webpage.
  3. Complete the following information:
    • Select a username (we suggest using your BruinMail address as a username)
    • Banner ID (S Number) *Include the “S” in your S Number
    • Email (Use your SLCC affiliated email,
    • First and Last Name
    • Date of Birth
  4. Click Submit

Having troubles setting up your account?

  • Be sure that you are entering your information correctly.
  • You will need to use your SLCC affiliated email address to successfully log into the portal.
  • If you’re already registered, the portal will not let you re-register. You will instead be asked to reset your password. Submit the email your account is affiliated with (your SLCC email) and you will immediately be sent an email to create a new password. Once you’ve created a new password, visit the portal and try again.
  • If you are still having trouble logging in, call the Center for Health & Counseling at 801-957-4268 during office hours to obtain assistance.


If you’re new to the CHC’s services, it’s likely that you need to fill out paperwork prior to your appointment.

When accessing the Patient Portal, you should be able to view your upcoming appointment(s) as shown below. If you don’t see an upcoming appointment on your account, call the Center for Health & Counseling to schedule an appointment. If you have paperwork to complete prior to your appointment, you will see how many forms that need to be completed in green as shown in the example below.

Attention! Appointments will be rescheduled if new patient paperwork is not completed prior to the appointment date. Thank you for abiding by our policies!
