Perkins V Needs-Based Assessment – Informed Consent Statement
You are being asked to take part in a survey of career and technical education at Salt Lake Community College. We are asking you to take part because of your affiliation with one of the K-12 schools, technical schools, and/or Salt Lake Community College within the Wasatch Front South Region. Please read this form carefully and ask any questions you may have before agreeing to take part. All questions can be directed to CTE@slcc.edu. This survey should take approximately 15-30 minutes to complete.
What is this project about: The purpose of this study is to identify educational gaps in career and technical education programs at Salt Lake Community College as required in part by the Strengthening Career and Technical Education for the 21st Century Act (Perkins V).
What we ask you to do: If you agree to participate, this will be conducted in an online survey. The questions will be about CTE programs of study; are these programs adequate to meet the needs of students and industry, are the programs equitable, are the programs sufficient in size, scope and quality.
I do not anticipate any risks to you participating in this study other than those encountered in day-to-day life.
There are no benefits to you. The goal of this survey is to continue to improve upon career and technical education at Salt Lake Community College.
There is no compensation for participating in this survey and/or forum.
Your answers will be confidential
The records of this study will be kept private. In the Comprehensive Local Needs Assessment Report we make public we will not include any information that will make it possible to identify you. Research records will be secured following SLCC institutional data security methods; only the researchers will have access to the records.
Taking part in this study is completely voluntary. You may skip any questions that you do not want to answer. If you decide not to take part or to skip some of the questions, it will not affect your current or future relationship with Salt Lake Community College. If you decide to take part, you are free to withdraw at any time.
