Financial aid forms can be found on the MySLCC Financial Aid card. Forms are available for multiple academic years. Complete forms for each academic year you want to receive financial aid.

Independent Status Appeal: Students who do not qualify for independent status on the Free Application for Federal Student Aid (FAFSA) may request a review of their dependency status. Students wishing to be considered independent must demonstrate that they have unusual circumstances. Students must complete the FAFSA before requesting this appeal and meet with a financial aid advisor to discuss their circumstances. Professional Judgment (Special Circumstance) Appeal: Students who have experienced a significant change in their financial status may request special consideration. It is recommended that students meet with a Financial Aid Advisor to submit this form. Students must complete the FAFSA (Free Application for Federal Student Aid) before requesting this appeal and meet with a financial aid advisor regarding their circumstances. Satisfactory Academic Progress (SAP) Appeal: Students who do not meet SAP standards after their warning term may submit an appeal. Appeals are reviewed by a committee to determine if the documented circumstances warrant an extension of aid during a probation period. Information on SAP can be found here. Cost of Attendance (COA) Appeal: Students whose expenses are higher than anticipated may request to have their cost of attendance increased. If their appeal is approved, they may apply for additional financial aid – usually student loans. It is recommended that students meet with a Financial Aid Advisor to submit this form. Students must complete the FAFSA (Free Application for Federal Student Aid) before requesting this appeal and meet with a financial aid advisor regarding their circumstances. Enrollment Exception Request: This form is used to request an exception to the minimum credit requirement for a scholarship and/or tuition waiver. These are reviewed on a case-by-case basis and there is no guarantee it will be approved. Approvals can take up to 7 business days to be determined. | Form |
SLCC Scholarship/Tuition Waiver Deferment Request Form: This request allows students who will be taking a leave of absence from school to request their scholarship be held on their behalf for a set amount of time. With the variety of rules and stipulations set by award providers, we cannot guarantee approval, and they will be reviewed on a case-by-case basis. | Form |
SAP Appeal for Scholarships/Waivers: This is for scholarship/waiver consideration only. If you must also appeal for Federal financial aid (FAFSA) as well, do not use this form and fill out the regular SAP appeal which can be found on this page. This appeal allows students that have lost their eligibility for scholarship/tuition waiver to explain circumstances that prevented them from meeting SAP standards during previous semesters. The outcome of the appeal will depend on the nature of the circumstances, the quality of the documentation, and the student’s plan for future success. | Form |
Financial aid can cover up to 150% of the hours required to complete a degree or certificate program. When evaluating maximum time frame, we must consider your full academic record at Salt Lake Community College, including transfer hours, regardless of whether you received financial aid during those terms.
Evaluation is required when you have attempted:
- 78 hours if seeking an Associate of Pre-Engineering (APE)
- 74 hours if seeking an Associate of Applied Science (AAS) or Diploma (DI)
- 72 hours if seeking an Associate of Science (AS) or Associate of Arts (AA)
- 35 hours if seeking a Certificate of Completion (CC)
If you have been informed that you are near the maximum number of allowed credit hours, you will need to submit a Maximum Time Frame Audit Request. This will be sent to your Academic Advisor, who will send your completed audit to us for review. You will be notified of the results via Bruinmail. This process can take 2-3 weeks.
Maximum Time Frame Audit FormIf you submitted an audit and have been informed that your evaluation has been denied, or you have exceeded your maximum time frame, you will need to appeal to extend your maximum time frame if you need more credit hours to graduate. Please read the instructions thoroughly. These appeals are reviewed on a case-by-case basis and are subject to approval.
Appeal to Extend Maximum Time Frame**Please note that changing your program does not reset your maximum time frame. You will need to submit a new audit for any program changes.
