Scholarship Forms

These forms are for students who have already been awarded a scholarship or tuition waiver. They allow recipients to request exceptions, deferments, or appeal eligibility issues. All submissions are reviewed case by case and require supporting documentation. Submitting form does not guarantee approval.

  • Enrollment Exception Request: This form is used to request an exception to the minimum credit requirement for a scholarship and/or tuition waiver. These are reviewed on a case-by-case basis and there is no guarantee it will be approved.
  • Scholarship Deferment Request Form: This form is used to request that a student’s scholarship (i.e. Opportunity, Regents, T.H. Bell, etc.) be held for a designated period during student’s leave of absence. Deferment is not guaranteed but will be given due consideration of all the required documentation is submitted within a reasonable period of time for a student’s leave of absence begins. Approved deferments extend your eligibility period but do not increase award amounts. If your leave lasts longer than expected, an updated deferment request is required.
  • SAP Appeal for Scholarships/Waivers: This form is used by students who have lost eligibility for scholarships and/or tuition waivers to explain the circumstances that prevented them from meeting Satisfactory Academic Progress (SAP) standards in prior semesters. The outcome of the appeal is based on the nature of the circumstances, the strength of the supporting documentation, and the student’s plan for academic improvement.     • Notice: Students who have an SAP Appeal for Scholarships/Tuition waiver approved will not have their federal aid reinstated. Students that are ineligible for federal aid due to SAP MUST successfully complete a Satisfactory Academic Progress (SAP) Appeal as described on the Financial Aid Forms page.
